This agreement applies to all breathwork sessions, classes, workshops, and events, online or in-person, facilitated by Mattias Dylan Horseman, Finding Magik LLC, or in collaboration with other facilitators, guest teachers, or partners, unless otherwise specified in writing.

Informed Consent and Assumption of Risk and Release of Liability: Client certifies that they are in adequate physical, emotional, & mental condition to participate and engage in the practice. You are also fully aware and understand that the nature of the Breathwork sessions may involve physical movement, emotional release, and the potential for altered states of consciousness and you, as the client, assumes full responsibility for your participation and releases Finding Magik LLC, Mattias Dylan Horseman, and affiliates, contractors, and/or employees/volunteers from any liability for physical, emotional, mental, or spiritual injury resulting from participation in Breathwork sessions.

Health and Well-being Acknowledgment: Client confirms that they are in good physical health and mental condition, and that they do not suffer from any condition that would hinder their participation in Breathwork activities, including but not limited to: cardiovascular conditions including high or abnormal blood pressure, history of aneurysms, heart attack or stroke, heart disease, uncontrolled thyroid conditions and uncontrolled diabetes, epilepsy or history of seizures, severe asthma, glaucoma, detached retina, osteoporosis, recent surgery, diagnosed severe PTSD, severe mental health conditions such as psychosis, schizophrenia, bipolar, any person with mental health concerns who is not in treatment or lacks adequate support, any person experiencing an emotional or spiritual crisis. Any person that is pregnant. This list is not exhaustive. If you are unsure whether Breathwork is appropriate for you, it is your responsibility to consult with a qualified healthcare provider prior to participation. and we generally recommend if you have a question about a condition you may have that is not listed, that you consult a physician before beginning Breathwork. Client acknowledges that participation is voluntary and releases Finding Magik LLC, Mattias Dylan Horseman and affiliates, contractors, and/ or employees/volunteers from liability for any risks, injuries, or damages.

Media & Recording Consent: I understand that group breathwork sessions may be recorded (video and/or audio) for recap, livestream, or promotional purposes. I give permission to Mattias Dylan Horseman and Finding Magik LLC to use my image, voice, or likeness for these purposes. If I prefer not to be included, I will notify the facilitator in writing before the session.

For individual sessions, I consent to audio recordings being made for private integration purposes only, shared exclusively with me. These recordings will not be distributed publicly.

Participation in group and group online sessions (hosted via streaming services) implies consent to recording unless otherwise communicated in advance. Cameras may be turned off at your discretion unless otherwise requested for safety and facilitation purposes.

Indemnification: Client releases and waives any claims against the facilitators of the Breathwork session, including Finding Magik LLC, Mattias Dylan Horseman, and affiliates, contractors, and/or employees/volunteers, and agrees to indemnify and hold them harmless for any injuries or illnesses experienced during the session.

ACKNOWLEDGMENT: I, the undersigned, hereby acknowledge that I have read and understood the terms of this release and voluntarily agree to its terms and conditions. By submitting this form electronically, I acknowledge that my typed name serves as my digital signature and confirms my understanding and agreement to the above terms.
