Informed Consent and Assumption of Risk and Release of Liability : Client certifies that they are in adequate physical condition to participate in physical exercise and are fully aware of the nature of the Breathwork session. Client assumes full responsibility for their participation and releases Finding Magik LLC, Mattias Dylan Horseman, and affiliates, contractors, and/or employees/volunteers from any liability for physical injury resulting from participation in Breathwork sessions.
Health and Well-being Acknowledgment : Client confirms that they are in good physical health and mental condition, and that they do not suffer from any condition that would hinder their participation in Breathwork activities, including but not limited to: cardiovascular conditions including high or abnormal blood pressure, history of aneurysms, heart attack or stroke, heart disease, uncontrolled thyroid conditions and uncontrolled diabetes, epilepsy or history of seizures, severe asthma, glaucoma, detached retina, osteoporosis, recent surgery, diagnosed severe PTSD, severe mental health conditions such as psychosis, schizophrenia, bipolar, any person with mental health concerns who is not in treatment or lacks adequate support, any person experiencing an emotional or spiritual crisis. Any person that is pregnant. This list is not all inclusive and we generally recommend if you have a question about a condition you may have that is not listed, that you consult a physician before beginning Breathwork. Client acknowledges that participation is voluntary and releases Finding Magik LLC, Mattias Dylan Horseman and affiliates, contractors, and/ or employees/volunteers from liability for any risks, injuries, or damages.
Media Release : Client agrees to the use of their image for promotional purposes, including but not limited to, photos and videos taken during group activities. Client grants Finding Magik LLC and Mattias Dylan Horseman permission to use their likeness for such purposes without compensation.
Indemnification : Client releases and waives any claims against the facilitators of the Breathwork session, including Finding Magik LLC, Mattias Dylan Horseman, and affiliates, contractors, and/or employees/volunteers, and agrees to indemnify and hold them harmless for any injuries or illnesses experienced during the session.
